East Cheshire NHS Trust - Hospital Discharge Community Support Guide

At East Cheshire NHS Trust we usually have around 250+ volunteers supporting Macclesfield District Hospital, Congleton War Memorial, Knutsford and District Hospital and our community colleagues.

At Macclesfield hospital we plan to offer a light touch volunteer telephone support and signposting service to regularly check on and assist people after hospital discharge through follow-up phone calls. Once patients have returned home our hospital discharge volunteers will directly refer and signpost anyone who needs assistance at home to find the help they need during their recovery. It may be that help is needed with:

· reducing energy bills· welfare and benefits advice

· financial advice and saving money

· adapting the home (purchase or short-term loan of specialist equipment e.g. emergency pendants, mobility aids, wheelchairs,)

· additional home care assistance (e.g. help with daily tasks like washing, getting dressed, cooking, cleaning, food shopping, collecting prescriptions and medication),

· boiler repairs,

· appliance repairs,

· DIY,

· befriending

· carers advice

· managing someone else’s finances

· setting up a will / (LPA) Lasting Power of Attorney

This is not an exhaustive list. Most likely this support will come from local voluntary and community-based groups and organisations.

We will be creating a discharge pack of resources for the discharge volunteers who will be offering telephone support to patients as a reference. The guide will include a basic directory of local services to help people manage their condition and recovery at home.
